ISMP Canada recruiting patients for a focus group on medication errors

Have you ever experienced a mistake with your medication? Received someone else’s prescription? Received the wrong dose of your regular medications? Took your medication incorrectly?

The Institute for Safe Medication Practices (ISMP) Canada is an independent not-for-profit organization that works towards preventing harm from medications and would like to hear from patients and families about your experience with medications.

They are inviting patients and caregivers to be part of a focus group to better understand what patients and caregivers know or would like to know about staying safe with their medication. What would you do if you experienced a medication error? We would like to have a conversation about medication errors and how ISMP Canada can reduce the risks.
